Emergency Contact Information Signature Form

As we have noted in the online registration process, we will need your physical signature to complete the Emergency Contact Information Form.  This can be done in the parish office, office of youth ministry, parent meetings, or at youth ministry events or by clicking this link.  If you have printed and signed the signature form you can drop it off at the parish office, drop it in the Sunday Collection, or mail it to the parish (11441 Hague Rd, Fishers IN, 46038; Attn:  Office of Youth Ministry)
